New Patient Forms

We offer several ways for new patients to check in and complete new patient forms at our Rock Hill office. If you would like complete forms on a tablet computer in our office, come ten minutes early for your first appointment. Bring your insurance card, social security number, and notes on your health history. Your information will go from our secure notebook computer into our office’s practice manament system.

An office staff member can also sit down with you and enter your information in our system as she interviews you in our office. She will ask for your insurance and health information during this process.

If you would prefer to fill out forms in advance, click below to download new patient forms. Email or fax the forms to us and we will enter them before your first appointment. You can also arrive a little early for your appointment with the completed forms.
